Your Best Reviews Won't Get You Cited by ChatGPT
That field is no longer the only one. When a parent today asks ChatGPT “who is a good orthodontist near me for my teenager,” the assistant does not return ten blue links. It returns one answer, sometimes with two or three cited sources. The parent may never see a list of practices at all.
Being cited is the new visibility. And based on audits of over 120 orthodontic practices completed since March 2026, most practices are not set up to be cited.
This is why we published The 2026 Citation Gap Report: to document, for the first time, how the profession actually looks to the AI answer engines that increasingly decide which practices get recommended.
What is the Citation Gap?
The Citation Gap is the distance between two layers of signals on a practice website.
The first layer is what patients see: photos, reviews, phone numbers, staff bios, the Google Business Profile. This is the visible layer, and orthodontic practices are strong here. Reviews are plentiful, response rates are high, and GBP completeness is the norm across the profession.
The second layer is what AI engines process: schema markup, structured content, entity relationships, semantic organization. This is the readable layer, and the profession is weak here.
The KAL™ AI Visibility Audit findings are consistent across the study. Patient Experience is the strongest category. Schema markup and Competitive Intelligence are the two weakest. The pattern holds practice after practice, market after market. Practices have optimized for humans and left the machine-readable foundation almost entirely untouched.
Why does this matter? Because AI answer engines like ChatGPT, Perplexity, Google AI Overviews, and Copilot cite sources. They cannot cite what they cannot parse. A perfect five-star Google review counts for nothing if the AI engine looking at your site cannot identify you as a Practice, cannot recognize your services as MedicalProcedure entities, and cannot pull authoritative statements about your credentials from structured markup.
Where does the professionstand right now?
The report groups practices into four bands based on their composite AI Visibility Score. Only a small minority of practices meet the AI Ready standard. Nearly half fall into the At Risk or Critical bands. The typical practice is Developing, which is the false-security zone. Practices in this band appear in AI results some of the time. They score well enough to be citable when conditions favor them. But they are not consistently recommended, and their visibility is fragile against any competitor who closes the gap.
The Schema category is particularly striking. About a third of practices audited have no schema markup on the site at all. A majority score in the lowest band. The majority fall below the threshold AI engines rely on to identify a source with confidence.
These are not opinions or projections. They are direct observations from over 120 audits conducted since March 2026 using the current KAL™ AI Visibility Audit framework.
Why is current AI visibility a borrowed advantage?
Some practices are appearing in AI answer results today without having done any specific AI optimization work. That visibility is not accidental, but it is temporary.
AI answer engines currently lean heavily on traditional search signals to identify authoritative sources. Practices with strong SEO histories, established domain authority, and well-optimized on-page content are being surfaced in AI results because of that legacy foundation. That advantage is closing. Google search is not going away. That needs to be said clearly. Traditional search remains the primary discovery path for many high-intent orthodontic queries, and it will continue to be for the foreseeable future. AI visibility does not replace SEO. It adds a new discipline on top of it. Practices that already invest in SEO are ahead of the curve on foundation. They just have to build the AEO layer next.
What if my practice is already being recommended by AI?
This is the most common question we hear from practice owners after we walk them through the report findings. And it is worth answering directly.
If your practice is already being recommended by AI answer engines, that is good news. It means the foundation you built with SEO and reputation management is doing some work for you in the new environment. Enjoy the moment.
But here is the harder truth. Practices that show up in AI results today often show up by default rather than by design. There simply are not enough optimized competitors in your local market yet to displace you. The engines are pulling from what they have, and what they have is you.
That situation does not last. One optimized local competitor can displace an undefended practice in AI answer results, and the displaced practice usually never knows it happened. There is no notification. No email. Just fewer inbound calls, and no clear reason why. This is what we call the false security gap: the distance between being visible today and being defensibly visible tomorrow.

How should practices respond?
