The AI Referral Shift in Healthcare

Patients are increasingly bypassing traditional search engines and asking AI platforms to recommend healthcare providers directly. The tools driving this shift include ChatGPT (900 million weekly active users as of February 2026), Google AI Overviews (appearing on nearly half of all U.S. searches), Perplexity AI, Gemini, and voice assistants like Siri and Alexa.

The fundamental difference: AI does not return a list of ten links and let the patient decide. It synthesizes available information and makes a recommendation. Practices that appear in those recommendations get the phone call. Practices that do not are removed from consideration before the patient ever visits a single website.

Healthcare is the most impacted industry. According to a WebFX study of 2.37 million U.S. search queries, healthcare queries trigger AI-generated answers at a rate of 51.6%, the highest of any industry. By December 2025, 88% of healthcare-related Google searches produced an AI Overview before displaying traditional results, according to Search Engine Journal.

The “False Security” Problem

KaleidoscopeAI™ identified a critical market dynamic during pre-launch research. In many local markets, some orthodontic practices currently appear in AI recommendations by default, simply because no competitor in their area has optimized for AI visibility. These practices may assume they are safe.

This visibility is incidental rather than earned. The moment a single competitor invests in AI visibility optimization, the practices that were merely present get displaced. Because there is no ranking report or notification for AI recommendations, practices may never realize they have been displaced until new patient numbers decline.

KAL was built to address both scenarios: making invisible practices visible to AI and making currently visible practices defensible against future competitors.

How KAL Works

KAL measures how well AI systems can find, understand, and recommend an orthodontic practice using a proprietary AI Visibility Score on a scale of 0 to 100. The system evaluates five core areas:

Schema markup completeness: Does the practice’s website contain structured data that AI can interpret?

NAP consistency: Is the practice’s name, address, and phone number consistent across 60+ directories?

Content architecture: Does the website content read like clinical expertise, or generic marketing language?

Third-party authority signals: What do patient reviews, professional directory listings, and local media mentions tell AI about the practice?

Live AI search performance: Does the practice actually appear when patients ask ChatGPT, Gemini, or Google AI who to call?

After auditing dozens of orthodontic practices across major U.S. markets, KAL found that the average AI Visibility Score sits below 40 out of 100. Most practices, including those with strong Google rankings, are largely invisible to AI.

What surprised us most during our pre-launch audits was how many practices with excellent Google rankings had almost no AI visibility. These are practices spending thousands of dollars a month on digital marketing, and AI cannot find them. Only about 17% of sources cited in AI-generated answers also rank in the organic top 10. That means 83% of AI recommendations are going to practices that traditional SEO alone would never surface. KAL closes that gap.
